First of all, download Nifskope here.
Open Nifskope, and load the baked mesh you want to work with.
Locate the textures icon between all the mumbo-jumbo. It looks like this:

Right-click on it, and choose the Texture menu, and then Export, like this:

You'll be prompted to select a destination folder and a file name (it only extracts textures as .tga).
When you have a new texture you want to add to the mesh, right-click again on the texture icon
and this time select the menu Textures\Choose, like this:

Another window will let you select the new texture. The only thing left now is to save the modified mesh somewhere,
and you're set!
